Health Office information:
Pre-Participation Athletic Physical Forms:
A current physical (and medical clearance) is required in order to participate in NJ Junior ROTC at MAST, including physical training, drill and formation. It is also required to participate in the vessel trips. Physicals are good for 365 days from date of medical examination. Please complete health history page prior to Medical examination. Call my office if you need to know when your child's physical expires.
Pre-participation athletic physical
Seasonal Concerns: It's allergy and sun safety season. Please wear sunscreen if you are going on the boat or going to the beach. Hats and long sleeve shirts are also important. Take allergy medication at home if indicated.
Illness:
Any student with a temperature of 100° or higher, vomiting, diarrhea, or signs and symptoms of a possible communicable disease, will be sent home. Please keep students home until they remain fever-free for 24 hours without the aid of medication and free of vomiting or diarrhea symptoms for 24 hours prior to returning to school. All children with suspicious rashes must be excluded from school until the rash has been determined non-contagious by a doctor.
Injuries:
If a student requires the use of crutches, braces, splints, wheelchairs, etc in school, a medical note is required. If a medical note is received exempting a student from ROTC activities, the student is also exempt from vessel trips unless clarified by the Primary Medical Provider.
